Ogden Clinic is offering the COVID-19 vaccine at the below locations only.

Instructions For ALL Locations:
What do I need to bring?:
- 1. Print and complete online forms found here or en Español.
- 2. Your insurance card.
- 3. A face covering (mandatory).
Plan to stay the clinic 15-30 minutes after your vaccination for observation for adverse reactions, which are extremely rare. We will make your second dose appointment while at our office.
