Schedule a Telemedicine Appointment

To schedule a Telemedicine visit, please call your provider's office. Below is a drop down list of our providers that are currently offering telemedicine appointments for some of their patients.

Prior to your scheduled appointment, you will receive a call ID number from your provider’s office along with the website to join your provider’s waiting room.

This form should not be used for urgent care matters or potentially life-threatening conditions.

In those situations, please call 911 or go to the nearest urgent care clinic or emergency room.

You Will Need The Following:

Webcam & Microphone
  • Smartphone
  • Computer or Laptop
  • Tablet or iPad

 

Internet Connection
  • Strong WiFi
  • Cellular data
  • Ethernet cable

Close Unused Tabs
  • If using a laptop or tablet, close out other open tabs or streaming services.
  • It may also be helpful to turn off your smartphone and restart it prior to your call.
  • While in the waiting room, avoid switching between apps.
  • If you run into poor connection during your appointment, refresh the window.
Also Helpful
  • Another person. It's good to have someone on standby to hold the phone/device, just in case your provider needs to assess movements or needs an unobstructed view of the ailment.
  • Good light and limited background noise.
  • Headphones and a microphone if you have them. This will help you and your provider sound clearer.
  • Limited motion: Prop your phone or tablet on something sturdy or use a stand rather than holding the device.

Ready for Your Appointment? Here is How to Check-In

  1. Click on the link that was texted/emailed to you by your provider's office. If they have not sent you the link, please contact your provider's office

    By agreeing to the terms of service you are agreeing to Ogden Clinic's Consent for Treatment and Billing Agreement.

  2. From the drop-down list, select the provider you need to see.
  3. Type in your name when prompted, then check into the provider’s waiting room.
  4. You are now inside your provider’s waiting room. They will begin the call shortly.
