To request an appointment, please complete the form and we will contact you within 24 hours or the next business day.

This form should not be used for urgent care matters or potentially life-threatening conditions.

In those situations, please call 911 or go to the nearest urgent care clinic or emergency room.

Due to the potential complexity and nuance of some types of appointments, please call us if you need one of the following appointments:

Yearly Physicals or Wellness Exam
An appointment in the next 30 minutes

NOTE: Upon booking an appointment, you will receive an email/text notification from Healow indicating your appointment has been confirmed. This is a confirmation that your requested appointment has been reserved, but the appointment may still need additional review by our Patient Services team. Once your appointment has been completely confirmed, our office will send an additional confirmation via text/phone message with your appointment details.

COVID-19 Appointment Notice

You must be currently experiencing symptoms to have a COVID-19 test. Ogden Clinic is not testing asymptomatic patients until further notice.

Upon scheduling, your appointment may take place five(5) days from exposure, which is when test results have the best chance of accuracy.
